Somerset County Council in Bridgwater is seeking an enthusiastic practitioner to support young people and families as they move from Children’s Services into Adult Social Care. You will build positive relationships, assess needs under the Care Act, and develop personalised plans to promote independence and choice.
You will work under guidance to coordinate services, participate in planning meetings, and contribute to a joined-up pathway into adult services for young people and carers.
