Monday to Friday 9-5 no weekends
We are seeking a dedicated and experienced Registered Nurse to fill the role of Nurse Case Manager. In this position, you will be responsible for overseeing complex care packages and ensuring the delivery of clinical excellence across our services in Yorkshire.
Key Responsibilities:
- Clinical Management:
Manage and oversee complex care packages, ensuring they are delivered in a holistic and clinically effective manner.
- Care Planning:
Develop and regularly review individualized care plans and risk assessments tailored to clients’ needs.
- Training & Leadership:
Lead clinical training sessions and conduct competency assessments for your care team to ensure high-quality service delivery.
- Collaboration:
Attend multidisciplinary team (MDT) meetings and engage with commissioning groups to facilitate coordinated care.
- Quality Assurance:
Perform audits on care notes and medication records to maintain compliance and high standards of care.
Qualifications &
Skills Required:
- Nursing Registration:
Must hold a valid NMC pin as a Registered Nurse.
- Clinical
Experience:
Previous experience in high dependency or critical care settings is essential.
- Specialized
Skills:
Experience with tracheostomy and ventilation in a community setting is highly desirable.
- Communication
Skills:
Strong communication and organizational skills, along with proficiency in IT.
- Regulatory Knowledge:
Understanding of Care Quality Commission (CQC) regulations.
- Transportation:
Must be a car owner/driver to manage client visits effectively.
