Advanced Clinical Practitioner – Home Visiting

Permanent | Up to 37.5 hours per week

Bring advanced urgent care to patients who need to be seen at home.

Not every patient who needs urgent care can come to us.

Some are housebound, frail, approaching the end of life or have been clinically assessed as needing a face-to-face review in their own home.

What they need is an experienced clinician who can assess the whole picture, make confident autonomous decisions and put the right care in place.

At IC24, we're looking for an Advanced Clinical Practitioner to join our Home Visiting Service across Sussex. You will have access to IC24’s fleet of vehicles and any equipment required to carry out this role, we only need you to bring your clinical expertise.

You'll work autonomously in patients' homes, using advanced assessment, examination, diagnostic and prescribing skills to provide safe, effective and patient-centred urgent care.

You'll care for patients across the full General Practice population, including children, older people, frail patients and those with mental health or end-of-life needs.

What you'll focus on

→ Assessing and treating patients in their own homes

You'll provide face-to-face assessment, diagnosis and treatment for patients who have been clinically triaged as needing a home visit.

You'll take a detailed history, carry out a physical examination and use advanced clinical reasoning to establish a differential diagnosis and agree the most appropriate treatment or care plan.

You'll see a wide range of urgent and primary care presentations, often without immediate access to the investigations and resources available in a hospital or clinic.

→ Making confident autonomous clinical decisions

Home visiting requires strong clinical judgement.

You'll often be assessing patients you've never met before, sometimes with limited information and sometimes in complex or emotionally difficult circumstances.

You'll need to be comfortable managing uncertainty, prioritising clinical risk and deciding whether someone can be safely treated and supported at home or needs escalation to another service.

You'll always work within your own competence and know when to seek further clinical input.

→ Treating the whole patient

Seeing someone in their own home gives you a different perspective on their health.

You'll care for patients across the full spectrum of General Practice, including newborns and children, adults, older and frail patients, people with mental health needs and those approaching the end of their lives.

You'll consider the patient's physical health alongside their environment, support network and wider social circumstances.

That means looking beyond the immediate symptoms and understanding what the patient genuinely needs to remain safe.

→ Prescribing and managing treatment at home

As an independent prescriber, you'll develop and deliver evidence-based treatment plans.

You'll prescribe safely within agreed clinical guidance, review existing medication where appropriate and make sure patients and carers understand what happens next.

Where required, you'll undertake point-of-care testing, arrange appropriate investigations and interpret available clinical information to support your decision-making.

→ Keeping patients safely in their community

One of the most important parts of home visiting is helping patients receive the right care in the right place.

Where clinically appropriate, you'll help patients remain safely at home by putting appropriate treatment, referrals, advice and safety-netting in place.
