Main Duties of the Job The post holder will be responsible for delivering a range of nursing services within their scope of practice, including:

• Supporting the management and monitoring of patients with long-term conditions.
• Undertaking chronic disease reviews, health assessments and care planning.
• Promoting patient self-management and healthy lifestyle choices.
• Supporting recall systems and annual review programmes.
• Monitoring and interpreting clinical results within competency levels.

• Providing evidence-based nursing care in accordance with local and national guidelines.
• Maintaining accurate, comprehensive and contemporaneous clinical records.
• Working collaboratively with GPs and the wider multidisciplinary team.
• Identifying and acting upon safeguarding concerns in line with practice policy.
• Contributing to quality improvement activities and preventative healthcare programmes.

• Phlebotomy.
• Vaccinations and immunisations.
• Cervical cytology (where appropriately trained).
• Wound care and dressing management.
• ECGs.
• Health promotion and screening programmes.
• NHS health checks and preventative care initiatives.
• Infection prevention and control procedures.
