Location: Brighton and Hove
The Mio Care Group in Oldham is seeking a Transfer of Care Officer (Community Reablement) on a permanent, full-time basis. The role sits in the Transfer of Care Hub at Royal Oldham Hospital, assisting with referrals and timely triage after discharge from acute care.
You will work with a multidisciplinary team to triage assessments and arrange appropriate placements, promoting independence while aligning with CQC standards and Mio Care policies.
