DescriptionAs described, Concept Rehab is a therapist-owned organization committed to providing quality rehabilitative care and professional development for staff and patients alike.
Responsibilities- Evaluate patients for rehabilitation services within 24 to 48 hours of physician’s orders.
- Develop effective treatment plans and administer treatment according to the plan of care.
- Communicate with physicians, clinical managers, other healthcare team members, patients and families regarding patient progress, problems and plans.
- Participate in Care Conference Meetings and Interdisciplinary Team Meetings.
- Provide in-service training for facility staff on restorative and functional maintenance programs.
- Record evaluations, treatment notes and progress notes; provide objective input for discharge summaries; and complete daily billing of services per policy and procedure.
- Instruct patients’ families or nursing staff in maintenance programs.
- Participate in discharge planning.
- Achieve or exceed the minimum productivity standard established by CRI.
- Recommend equipment as needed to improve delivery of clinical services.
- Report equipment problems to appropriate personnel.
- Act as a clinical supervisor for affiliating Speech/Language Pathology Graduate Clinicians for area universities and colleges.
- Comply with state-specific Speech Pathology Licensure Standards.
- Maintain an organized working environment and uphold Concept Rehab values and professional image.
- Undertake special projects and other duties as assigned.
- Master’s Degree in Speech-Language Pathology
- Certified by the American Speech-Language-Hearing Association (ASHA).
- Licensed in the state(s) where services are provided to practice as a Speech-Language Pathologist.
Strong relationship and interpersonal skills, high emotional intelligence, excellent oral and written communication, problem-solving and analytical abilities, technology proficiency, cultural awareness, ethical practice, and the ability to exercise independent judgment and discretion. Ability to maintain confidentiality.
