• Evaluate patients for rehabilitation services within 24 to 48 hours of physician’s orders.
• Develop effective treatment plans and administer treatment according to the plan of care.
• Communicate with physicians, clinical managers, other healthcare team members, patients and families regarding patient progress, problems and plans.
• Participate in Care Conference Meetings and Interdisciplinary Team Meetings.
• Provide in-service training for facility staff on restorative and functional maintenance programs.
• Record evaluations, treatment notes and progress notes; provide objective input for discharge summaries; and complete daily billing of services per policy and procedure.
• Instruct patients’ families or nursing staff in maintenance programs.
• Participate in discharge planning.
• Achieve or exceed the minimum productivity standard established by CRI.
• Recommend equipment as needed to improve delivery of clinical services.
• Report equipment problems to appropriate personnel.
• Act as a clinical supervisor for affiliating Speech/Language Pathology Graduate Clinicians for area universities and colleges.
• Comply with state-specific Speech Pathology Licensure Standards.
• Maintain an organized working environment and uphold Concept Rehab values and professional image.
• Undertake special projects and other duties as assigned.

• Master’s Degree in Speech-Language Pathology
• Certified by the American Speech-Language-Hearing Association (ASHA).
• Licensed in the state(s) where services are provided to practice as a Speech-Language Pathologist.

Strong relationship and interpersonal skills, high emotional intelligence, excellent oral and written communication, problem-solving and analytical abilities, technology proficiency, cultural awareness, ethical practice, and the ability to exercise independent judgment and discretion. Ability to maintain confidentiality.
