- Ensure all activities performed align with the vision of Elara Caring’s leadership and the Home Health team.
- Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
- Assess assigned caseload of home health care patients and families to identify the physical, psychosocial, and environmental needs of patients, using clinical records, interdisciplinary team reports, and on‑site evaluations.
- Assume primary responsibility for caseload, including planning, coordinating, implementing, and evaluating the plan of treatment.
- Assess patient needs and obtain data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status and incorporate that data into the plan of care.
- Make the initial evaluation and re‑evaluate the patient’s nursing needs during each visit, making follow‑up visits according to patient, family, and facility needs.
- Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other personnel involved with patient care.
- Revise the plan of care in response to identified patient care issues and notify the Clinical Supervisor, physician, facility, and other team members.
- Current, unrestricted state RN license.
- 1+ year experience in a clinical care setting as a nurse.
- Able and willing to travel within branch/office coverage area.
- Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
- Must be able and willing to travel 50% of the time.
- Associate’s Degree or Bachelor’s Degree in Nursing is preferred.
- Previous Home Health or Hospice Experience is preferred.
- Previous experience with Home Care Home Base is preferred.
You will report to the Clinical Manager or Clinical Supervisor.
