- Oversees all aspects of Pharmacy operations ensuring accurate dispensing. Strives to improve the operational process to increase efficiency and decrease expenses
- Ensures all medications are accurately prepared, labeled and checked prior to leaving the pharmacy.
- Interprets Physician order and accurately enters/validates orders. Validates orders. Validates/enters all STAT orders within 15 minutes and all non-urgent orders within 1 hour.
- Participates in the Pharmacy Improvement Process.
- Contributes to Departmental cost saving initiatives. Ideas submitted to Pharmacy Manager.
- Ensures all chemotherapy preparations are checked by one Pharmacist and two other Pharmacy personnel.
- Resolves Pyxis issues.
- Effectively supervises Technicians, signing all checked work and providing corrective action when necessary.
- Maintains the security and perpetual inventory of controlled substances.
- Ensures all IV admixtures are prepared using aseptic technique
- Under the guidance of the Pharmacy Manager, oversees training of new staff/students.
- Monitors inventory and orders medications as needed to meet the needs of our patients.
- Participates in the control, use and monitoring of investigational drugs.
- Evaluates the patient’s drug therapy, clarifying dosage, drug interactions, allergies, contraindications, and therapeutic duplication with the Prescriber.
- Monitors for and completes Adverse Drug Reaction reports.
- Investigates and completes Medication Occurrence reports.
- Evaluates patient care and response to pharmacotherapy. Evaluation should include, but is not limited to renal dosing, IV to PO conversion, aminoglycoside and vancomycin dosing, enoxaparin and epoetin alpha conversion, pediatric weight based dosing, antibiotic dosing and selection. Documents interventions.
- Monitors lab values and ensures TPN appropriateness. Verifies that LIP is credentialed.
- Evaluates chemotherapy regimes, verifying regimens/dosing against diagnosis.
- Actively participates in at least one Committee.
- Discourages the use of patient own medications and non-formulary medications and completes a non-formulary form for each non-formulary medication.
- Monitors warfarin on each assigned patient and provides education when required.
- Provides evidences based drug information and teaching to our patients and customers.
- Functions as a preceptor to new employees and students.
- Completes all mandatory educational assignments on time.
- Attends staff meetings while on duty. Signs off on minutes if absent.
- Reads email and communication log daily. Initials communication log.
- Conducts a self assessment, develops professional practice goals and demonstrates progress to completion of those goals, documenting the results.
- Voluntarily participates in the education of the professional staff including case presentations as appropriate (not required)
- Functions as liaison to assigned patient care location, interacting with manager and staff to promote customer satisfaction through a team effort.
- Less than 6 unexcused absences as per Hospital policy.
- Limits personal cell and internet use.
- Arrives to work on time and does not depart early without approval.
- Practices Pharmacy in accordance with State and Federal statutes/laws.
- Performs all job functions assigned by the Pharmacy Manager/Clinical Coordinator.
