SummaryAssists individuals in maintaining an interactive process of informed decision-making about Long-Term Services and Supports. Serves a key role in coordinating the efforts of formal and informal caregivers on behalf of clients. Care Management is a person-centered service that values the consumer’s choices and rights. Performs all duties in a manner that fosters the achievement of the organization’s mission to identify choices and provide services to help people of all ages, abilities and incomes to live at home.

Education
- Bachelor’s degree in administration, social work, nursing, public health, psychology, counseling or gerontology or related field required.
