Overview
Position:
Independent Optometrist leasing space inside Sam's Club stores. You will take a leading role in patient care with respect to eye and vision care as well as general health and wellbeing. As an independent lease holder, you have autonomy over your practice setup and operations.
Launch your services in Sam's Club. You will establish your days of practice, hours of operation, patient schedule, and fee schedule. You may opt into additional support and equipment as available.

- Keep 100% of exam/professional fees
For a fair market value, you can opt into:
- Advanced equipment/instrumentation
- Administrative and/or Opt-Tech support services if state allows
- Doctor of Optometry (OD) from an accredited optometry school
- Valid state optometry license
- Ophthalmologist (MD or DO) from an accredited school
- Valid state ophthalmology license
- General Liability Certificate/Professional Liability Insurance Certificate
ME/VA license agreements are with Gumberg Asset Management Company.
