Center Well Home Health LPN (Home Health Licensed Practical Nurse)

You’ll provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working closely with a dedicated team of physicians and clinicians, you’ll develop and manage care plans that support recovery and help patients get back to the life they love.

• Provide professional nursing services under the direction of a RN in compliance with state laws and Center Well policies.
• Deliver skilled nursing interventions for treatment, rehabilitation, and preventive care using a holistic approach.
• Collaborate with RN Supervisor regarding treatment plan changes and communicate orders from physicians.
• Maintain patient contact and coordinate care with physicians, clinical managers, and team.
• Document care consistently and accurately in shift notes and visit records.

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
• Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.

Base pay: $44,600 - $61,400 per year. Pay per visit/unit: $28.00 - $40.00.
