Bris Doc Healthcare Services is recruiting a Frailty-ACE Clinical Practitioner to assess frail patients with urgent care needs and coordinate holistic management plans aligned to patient preferences. You will work with the wider Frailty-ACE team to manage diverse clinical presentations across age groups in urgent primary care, including tele-referrals and remote assessments as needed.
The role requires full NMC/HCPC registration or equivalent geriatrics training, plus a MSc in advanced practice.
