Bris Doc is seeking a Frailty-ACE Clinical Practitioner to work collaboratively within the Frailty-ACE to assess frail patients’ urgent care needs and coordinate holistic, person-centred management plans aligned with patients’ preferences and best interests.
The role covers a broad range of clinical presentations across age groups in urgent primary care, including coordinating referrals and maintaining sensitivity to capacity issues within the hosting acute trust.
