Job Description

Job responsibilities

Care and Quality:
To assess, plan, implement and evaluate individualized patient care in collaboration with the patient and their carers and the multidisciplinary team. Ensure patient privacy and dignity, maintain confidentiality and robust GDPR practice. Deliver care compassionately with dignity and respect. Adhere to the Hospice Equality, Diversity and Respect Policy. Respect rights, act in accordance with legislation and policies, and acknowledge diversity.

Administer medications as prescribed and monitor effects. Respond to admissions effectively and within standards. Prioritise workload to meet priorities without compromising quality. Support patients and families, respecting beliefs and personal circumstances. Engage in clinical governance, contribute to practice improvement and clinical audits. Ensure compliance with health and safety policies and risk management. Challenge unsafe practice and uphold safety standards. Report accidents, incidents and complaints as required.

Communicate effectively within the IPU and with other professionals, ensuring documentation is accurate and timely. Essential documentation to be in line with hospice policy, NMC standards and Information Governance.

Communication:
Communicate with patients and families in a sensitive, supportive manner; identify barriers to communication, consider cultural and personal preferences; maintain integrity and professionalism. Ensure timely communication with internal/external allied health professionals to coordinate care. Complete documentation accurately and timely. Help patients/relatives overcome language barriers and deliver concise shift handovers.

Teamwork:
Work as an open and honest member of the multidisciplinary team; develop supportive relationships; supervise Health Care Assistants and Volunteers; support IPU management and shift leadership; participate in debriefing, reflective case reviews and MDT meetings; contribute to service development and provide feedback to colleagues.

Personal and Professional Development:
Participate in annual performance reviews and mandatory training; maintain professional development, support NMC revalidation, and contribute to research/audit activities. Be flexible to clinical and operational changes and mentor new staff and students. Build leadership skills for IPU shift leadership and share expertise with MDT colleagues.

Additional Responsibilities (for Personal Development Planning when fully competent): to lead shift, monitor practice and competence for Bank staff, participate in training and mentoring, and monitor practice and competence within the IPU.
