As the Practice Education Facilitator (PEF) within the Integrated Discharge Service (IDS) at North Bristol NHS Trust, you’ll shape how our workforce grows, learns and delivers for our patients.
This is an exciting opportunity to influence service quality, support staff across all levels, and drive a culture of continuous learning and compassion. Working within a dynamic multidisciplinary environment, you will help ensure patients experience safe, timely and effective discharge planning.

Main duties of the jobIn This Role, You Will
- Lead education, training and development initiatives for IDS staff, including structured induction, competency development and ongoing coaching.
- Support staff at all levels to build confidence and capability in complex discharge planning.
- Work closely with IDS operational leads, ward teams and community partners to identify learning needs and deliver high‑quality educational support.
- Provide supervision, mentorship and assessment to ensure safe, effective and evidence‑based discharge practice.
- Promote a positive learning culture, champion best practice and contribute to quality improvement across the service.
- Strong teaching, coaching or facilitation skills.
- Excellent communication and interpersonal skills.
- Experience in discharge planning within an acute setting.
- The ability to work collaboratively with multidisciplinary teams.
- A passion for developing others and improving patient outcomes.
