Support Worker – Assisted Discharge
Location:
Bristol Royal Infirmary and community work – Monday 10am-6pm Community based, Tuesday-Friday 12pm-8pm Hospital based – Bristol Royal Infirmary.
Salary: £24,479 per annum pro-rata (£13.45 per hour);

Full UK manual Driving Licence and access to a vehicle;

Seeking a kind, reliable, and positive‑energy team member who will support patients transitioning from hospital to home. No formal care experience required – ideally patient, thoughtful, and able to make a person feel at ease.
Day‑to‑day responsibilities- Provide practical and emotional support including welfare calls, shopping, and home visits.
- Support adults after a hospital stay to help them recover safely at home.
- Respond to referrals from clinicians and community health professionals.
- Assess needs and follow a personalised support plan for each service user.
- A kind, calm, and confident approach with great people skills.
- The ability to work independently and as part of a team.
- A full UK driving licence and willingness to drive service vehicles.
- Basic IT skills and a commitment to following procedures.
