The Clinical Practitioner will provide exemplary care for their patients, focusing on meeting population health needs. This includes consultation with patients across a range of dispositions from 111, primary care, 999, Emergency Department, Emergency Treatment Centre, and routine or urgent needs. Clinicians will be focused on improving health inclusion and removing barriers to holistic healthcare for vulnerable, marginalised, and excluded groups.

• Carry out structured medication reviews for patients with complex medication needs, including polypharmacy and frailty.
• Provide medicines optimisation and reconciliation following hospital discharge and other transitions of care.
• Support repeat prescribing processes, respond to patient medication queries, and contribute to clinical audits.
• Identify and manage prescribing risks in line with MHRA guidance and national safety alerts.
• Act as a source of clinical supervision for pharmacy technicians and work alongside prescribing administration colleagues to support safe and effective prescribing.
• Assess, diagnose, plan, implement and evaluate treatment/interventions for patients presenting with undifferentiated diagnosis, complex needs, acute and chronic conditions.
• Integrate drug and non‑drug‑based treatment methods into management plans.
• Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self‑care.
• Provide face‑to‑face and remote clinical assessment/treatment and manage plans.
• Provide clinical support to other health care professionals and operational colleagues within the team.
• Carry out visits to patients’ homes as required and ensure robust safety netting and risk management for patients presenting to the service.
• Make effective referrals to system partners and other care providers to ensure holistic provision of care.
• Participate in team discussions to drive best practice using the best available evidence.
• Maintain accurate electronic written patient records and observe Data Protection Act responsibilities.

• Master’s degree in Pharmacy or equivalent.
• Registered with the General Pharmaceutical Council (GPhC).
• Experience of working in a patient‑facing role in a clinical pharmacy or pharmacy setting.
• Strong knowledge of medicines optimisation, polypharmacy and prescribing safety.
• Experience of working with IT systems Microsoft Office and Clinical Systems.
• Experience of following set procedures and protocols and working to quality standards.
• Experience of working in a busy environment and meeting deadlines.
• Current UK professional registration (NMC, HCPC, or Physician Associate Managed Voluntary Register).
• Advanced Clinical Assessment, Reasoning and Clinical Decision Making or Physical Assessment and Clinical Reasoning qualification or experience and willingness to work towards this qualification.
• BSc qualification in a Clinical field or Biomedical science or equivalent.

• Independent prescribing qualification or working towards one.
• Experience in mental health assessment.
• Working towards MSc in Advanced or Specialist Practice or evidence of accumulation of relevant experience/knowledge.
• Experience with Adastra and/or EMIS.
