Dorset Council’s Home First Occupational Therapy team plays a key role in safe and timely discharges from hospital and community settings, using the Discharge to Assess model to support recovery at home.
Occupational Therapists assess before and after discharge, work with hospital teams, families, and clients to plan supports, and review arrangements to maximise independence and minimise long‑term needs, while delivering care in the right place at the right time.
