* Deliver high-quality clinical pharmacy services to allocated hospital wards, undertaking daily medicines reconciliation, clinical chart checks, and discharge planning.
* Provide expert advice on dosage adjustments, therapeutic drug monitoring, and drug interaction management to medical and nursing staff.
* Ensure full compliance with medicines optimisation policies, NHS drug formularies, and clinical governance frameworks.
* Support dispensary operations, aseptic preparation services, and clinical audit initiatives within the pharmacy directorate. Essential Requirements:
* Full GPhC Registration as a Pharmacist.
* Post-graduate Diploma in General Pharmacy Practice (or equivalent clinical experience).
* Demonstrable NHS hospital pharmacy experience at Band 6 or Band 7 level.
* Independent Prescribing (IP) qualification is highly advantageous. Evolve Staffing Ltd Band7
