This is Band 6 post has been newly created to provide increased capacity for assessments of new referrals to the team, at a time of increasing demand. The successful applicant will have the opportunity to work with service users presenting with a variety of mental health conditions including organic and functional. As part of an experienced, skilled and supportive multi-disciplinary team the post holder will be acting as Key Worker to deliver evidence based interventions to service users and support to carers.

Main duties of the role:

• To facilitate the delivery of personalised care and support planning in line with 'Your Team, Your Conversation, Your Plan.
• To provide personalised interventions to support people in Later Life with mental health difficulties.
• To act as a Key Worker for people for whom you are delivering interventions.
• To deliver a range of activities and interventions to support carers.
• To provide mentoring and training for others in relevant practice areas.

• Mobile with the facility to move quickly across a geographically dispersed area where there is limited access to public transport.

• Diploma level/ Degree in Nursing
• Current registration with the NMC

• Relevant post
  -registration qualifications and experience

• Substantial post-registration experience working in adult mental health

• Experience gained in a variety of settings including people in Later Life
