Dream Medical is working with a friendly GP surgery In Bristol, who are looking for a long term chronic disease practice nurse to join their multi-disciplinary team on a permanent basis. The hours, which can be child-friendly, are flexible to suit the candidate's requirements. The post is based in a modern, purpose built practice in a central location.

For this role you will need to be experienced in Long term conditions including Chronic disease management, Spirometry, Asthma Checks and Diabetes ( Type 1 & Type 2 and foot checks )

Requirements:

• NMC Registration
• RGN Qualifications
• Practice Nurse experience
• Up to date certificates for all training
