Looking for an experienced Practice Nurse specialising in Chronic Disease Management Bristol Flexi days/hours to suit

Dream Medical is working with a friendly GP surgery in Bristol, who are looking for a long-term chronic disease practice nurse to join their multi-disciplinary team on a permanent basis. The hours, which can be child-friendly, are flexible to suit the candidate's requirements. The post is based in a modern, purpose-built practice in a central location.

For this role you will need to be experienced in long-term conditions including Chronic disease management, Spirometry, Asthma Checks and Diabetes (Type 1 & Type 2 and foot checks).

Requirements:

• NMC Registration
• RGN Qualifications
• Practice Nurse experience
• Up-to-date certificates for all training
