About the Role
An exciting opportunity for a motivated nurse to join the Diabetes Team at Bristol NHS Foundation Trust. You will play a key role in delivering high‑quality inpatient and outpatient diabetes services across a diverse and dynamic clinical environment, supporting a friendly and supportive team committed to safe, personalised, evidence‑based care.
Key Responsibilities- Manage a caseload of diabetes patients across inpatient and outpatient settings, delivering high‑quality, evidence‑based care tailored to individual needs.
- Undertake autonomous, independent clinical practice under the direct and indirect supervision of the Senior Enhanced Practitioner, using enhanced clinical judgement and decision‑making skills.
- Assess, plan, implement and evaluate care for patients with complex diabetes needs, supporting effective treatment plans and positive outcomes.
- Provide specialist diabetes advice, education and emotional support to patients, families, and carers, empowering self‑management.
- Initiate, review and modify diabetes interventions and treatment plans, including insulin and oral medications, within scope of practice and Trust guidelines.
- Work collaboratively with the multidisciplinary team to ensure coordinated care pathways, safe discharge planning and ongoing management.
- Act as a clinical resource within the diabetes service, promoting evidence‑based practice and supporting high standards of patient‑centred care.
- Maintain accurate, comprehensive and contemporaneous clinical records in accordance with Trust policies and professional standards.
- Participate in diabetes education and training programmes for healthcare professionals, contributing to a positive learning environment.
- Contribute to clinical audit, quality improvement, service development and research activities to support continuous improvement of diabetes care and patient experience.
- Demonstrate professional accountability, compassionate care and effective communication at all times, recognising limitations of practice and escalating concerns.
- Proven leadership experience and excellent organisational and leadership skills.
- Experience managing a caseload of diabetes patients.
- Strong communication skills and a compassionate approach to patient care.
- Ability to work autonomously within the scope of practice and under supervision of a Senior Enhanced Practitioner.
- Willingness to learn and contribute to a positive team culture.
- Prepared to work towards becoming a Non‑Medical Prescriber (if appropriate for the statutory profession).
