• Job Title: Hospital Social Worker
• Starting Salary: £38,220 - £41,771 per annum, plus £1,911 - £2,088 market enhancement pay, 5% operational allowance per year, and £1,000 welcome payment. Relocation and lease car terms apply – pro‑rata for part time. Pay award pending.
• Hours: 37 hours per week
• Location: Hertfordshire

You will help ensure safe and timely discharges from hospital by:

• Assessing the needs of patients and carers
• Arranging appropriate care packages
• Liaising with professionals and agencies
• Supporting person‑centred discharge planning

The role operates a 7‑day service and currently follows a 1‑weekend‑in‑8 rota across the Hospital Team. The majority of work is carried out through the Discharge to Assess model.

• Social Work degree
• Registration with Social Work England
• Driving licence (ability to drive and/or travel around Hertfordshire)
• English fluency – the ability to converse at ease with members of the public and provide advice in accurate spoken English. A sign language interpreter may be required for those with a signed language requirement.
• Disclosure & Barring Service (DBS) check required.
