Family Support Worker – Full‑time (35 hours, 5 days per week) within the South West Care Team.

We are looking to appoint a Family Support Worker to deliver a high‑quality family support service as part of our South West Care Team to families in the local area, at a time when they need it most. The role reports to the Family Support Manager and works in partnership with health, education and social care professionals.

Responsibilities
  • Provide needs‑led emotional, social and practical support to families where a child or young person has a life‑threatening or terminal illness.
  • Manage your own schedule, remaining flexible to the needs of families on your caseload.
  • Build supportive relationships and help people as they deal with grief, loss and change.
  • Provide bereavement support and maintain knowledge of processes of grief, loss and change.
  • Use child protection and information‑sharing rules and data protection guidelines to support families.
  • Work collaboratively with the team and volunteers, ensuring delivery of a high‑quality service.
  • Support fundraising colleagues by writing case studies and family updates.
  • Use IT (including MSOffice) confidently and maintain accuracy.
Qualifications & Experience
  • Experienced child health, education or social care professional.
  • Working in a community environment, with a recognised qualification in education, health or social care.
  • Practical and people‑oriented, able to work at a fast pace whilst maintaining accuracy.
  • Persuasive and open communication skills.
  • Practical knowledge of diversity issues affecting children, young people and their families.
  • Full current driver’s licence and willingness to work flexible hours.
  • Enhanced DBS disclosure required.
