Outreach and Sustainment Officer

Outreach & Sustainment Officer – Waltham Forest PAYE: £18.53 per hour Role Purpose We are seeking a dedicated and compassionate Outreach & Sustainment Officer to support individuals who are sleeping rough or at risk of homelessness across Waltham Forest. In this role, you will deliver proactive, trauma-informed outreach, engaging with vulnerable individuals to provide welfare checks, assess their needs, and connect them with housing, health, welfare, and specialist support services.

Your work will help reduce harm, improve well being, and support people to move away from rough sleeping. You will also provide ongoing tenancy sustainment support from the point an individual moves into temporary accommodation, supported housing, or independent living. This includes helping service users settle into their accommodation, build independence, access essential services, and maintain long-term housing stability. Working collaboratively with internal teams and external partners, you will contribute specialist knowledge of homelessness, rough sleeping, and complex needs to ensure coordinated, person-centred interventions that achieve positive outcomes and reduce risk.
