As a Community Care Officer, you'll work with adults who have learning disabilities and long-term health conditions, frailty, or other care needs. You will be working with people living in a variety of settings, including their own homes, supported accommodation, residential care and nursing environments.

Working within the Learning Disability South Team, you will play a key role in supporting adults with learning disabilities to live fulfilling, independent, and meaningful lives.

Working within Lincolnshire's strengths-based Conversation Model, you will:

• Complete Adult Needs Assessments and reviews under the Care Act 2014
• Develop strengths-based support plans that focus on individual outcomes
• Work alongside individuals, carers, families and partner agencies
• Promote independence, wellbeing and community connections
• Respond to safeguarding concerns and contribute to safeguarding enquiries
• Support people through periods of change, crisis and transition
• Manage a varied and rewarding caseload across your locality

The Learning Disability Team focuses on working with adults who require assessments, Decision Support Tool, Section 75 reviews, care planning, and ongoing interventions to meet their care and support needs. This often involves undertaking comprehensive assessments, developing person-centred support plans, and working collaboratively with families, carers, health professionals, and other partner agencies.

• In accordance with Part 7 of the Immigration Act 2016 (Fluency Duty), the ability to converse at ease with customers and provide advice in accurate spoken English is essential for the post.

Currently vacancy available: 1 full-time Community Care Officer – South Learning Disability Team. Based at The Vista, Spalding. This is a community-based role, and you will need access to a vehicle and be able to travel across the South Lincolnshire area. The team covers Sleaford, Spalding, Grantham, Stamford and Bourne. Hybrid working will be a feature of this role. You will be given a nominated contractual work base and you will be required to commute to this work base when necessary.

Working arrangements regarding hybrid working will be discussed as part of the appointment process.

Experience of working with adults with learning disabilities is desirable but not essential, as full training and support will be provided.
