Community Care Live is seeking an Assistant Family Practitioner to help children aged 0–19 and their families achieve positive, lasting change through early help and targeted support. You will work with families to identify goals, overcome challenges and access the right support at the right time.
In collaboration with a supportive team, you will deliver one-to-one, outreach and session-based support, signpost services and maintain records for service improvement and accountability.
