The Lewisham Care Partnership is seeking a compassionate Social Prescribing Link Worker to help patients access community resources to improve wellbeing and independence. You will work with patients, families, carers, and professionals to develop personalised care plans, connect with local services, and promote social prescribing across the Primary Care Network.
This multidisciplinary role focuses on reducing health inequalities, building community links, and evaluating outcomes within TLCP PCN.
