Adults Social Worker – Discharge to Assess Team within Discharge to Assess in Bristol, earning £35.00 Hourly. Ongoing. Full Time position available. This exciting opportunity allows you to make a real impact on people’s lives as part of a dedicated team supporting adults in a community setting. As a pivotal part of the Discharge to Assess Team, your role will be predominantly based in Bristol, working within the Community Hospital Discharge Service.

- Assess individuals’ longer-term care and support needs by conducting Care Act assessments and completing mental capacity assessments.
- Make best interest decisions and carry out safeguarding enquiries and risk assessments.
- Develop tailored care plans by working closely with individuals, families, and multidisciplinary teams to coordinate care arrangements.
- Support individuals in transitioning back into the community by working within a range of discharge pathways including Pathway 1, Pathway 2, and Pathway 3.
- Facilitate smooth discharges from acute hospitals and provide continuity of care as a Community Hospital Discharge Social Worker.
- Regularly engage in office-based duties in Bristol for two days a week while enjoying flexibility in your schedule.
