Angels Care Home Health is seeking a Licensed Practical Nurse (LPN) to administer skilled nursing care in clients' homes, under the supervision of a registered nurse. You will coordinate care with the interdisciplinary team, patients, families, and referring agencies to ensure safe, effective patient outcomes.
The role requires graduation from an accredited LPN program, an active state license, and a valid driver's license. Two years of LPN experience, preferably in home health, is preferred.
