Automatic Data Processing, Inc. is seeking a dedicated PACE Social Worker to provide direct social work case management to program participants, including intake, screening, counseling and referrals.
The PACE Social Worker will evaluate and coordinate new enrollees to the program. You will coordinate care with interdisciplinary teams, conduct home assessments, develop the Plan of Care, facilitate transitions to community services, and ensure HIPAA compliance and timely documentation.
