• Assess adult care and support needs and develop detailed, outcome-focused care plans
• Implement and regularly review support plans to ensure needs continue to be met
• Undertake adult safeguarding enquiries and contribute to safeguarding plans
• Provide guidance and advice to colleagues and partner agencies on safeguarding matters
• Promote independence using recognised social work methods, models and tools
• Make sound professional decisions in line with current legislation and policy frameworks
• Work collaboratively with service users, families and partner organisations
