The Pharmacy Clinical Specialist is responsible for the provision of direct patient care, training and education of other health care providers, contribution to formulary management activities, and participation in quality improvement projects. Rotating shifts include weekends and holidays.

ESSENTIAL RESPONSIBILITIES / DUTIES:
  • Review and verify medication orders, evaluate patient charts and laboratory data to identify or prevent drug‑related problems.
  • Collaborate with the central pharmacist and technicians to ensure accurate, timely acquisition, distribution and control of all medications, including investigational drugs.
  • Adhere to all protocols and policies related to controlled substances.
  • Supervise supportive personnel, including student interns and technicians.
  • Use pharmacy automation and/or barcoding systems to document checking of any medication or sterile product prior to dispensing.
  • Rotate through different clinical shifts with a specific core practice area.
  • Make recommendations to optimize pharmacotherapy within a multidisciplinary team and provide effective transition of care, including medication reconciliation and discharge counseling.
  • Perform daily targeted interventions to decrease risk of readmission and improve medication safety.
  • Serve as a pharmacy liaison to the BMC staff, ensuring connectivity between clinical and distributive services.
  • Preceptor for residents and students and assist in orientation and training of new pharmacists, residents, interns and technicians.
  • Participate in formulary management activities, including monograph preparation and medication use evaluations.
  • Identify opportunities for improvement in patient care or medication use and take an active role in quality improvement projects.
  • Maintain professional competency through continuing education and conform to Boston Medical Center’s standards of conduct.
JOB REQUIREMENTS
  • Doctor of Pharmacy (Pharm

    D) or Bachelor of Science in Pharmacy with an advanced degree.
  • PGY1 pharmacy residency or at least three years of equivalent work experience.
  • Valid license to practice pharmacy in the Commonwealth of Massachusetts (verified by hire).
  • Board Certification in Ambulatory Care Pharmacy (BCACP) or must obtain certification within one year of eligibility.
  • Strong clinical knowledge relevant to the practice area, effective project management and biostatistics application.
  • Excellent attention to detail and effective verbal and written communication skills; proficiency in English required.
  • Proficiency with Microsoft Office (Word, Excel, Outlook) and web browsers.
