Job Description - Services Coordinator Generalist (250008B4)
The Department of Developmental Services (DDS) is seeking a motivated and experienced human service professional for the role of a Part-Time Services Coordinator Generalist (Human Service Coordinator I) in the Brockton Area Office. The Services Coordinator Generalist monitors the safety and wellbeing of assigned individuals and advocates regarding their human, civil, and legal rights through the implementation of support services, the Individual's service plan, and other necessary means.
Dutiesand Responsibilities
- Monitor DDS funded and auxiliary funded programs.
- Maintain regular contact with individuals, families, and providers.
- Travel to individuals' homes, programs, and outside meetings; that may or may not be in proximity of the Area Office.
- Coordinate, facilitate, and monitor the development and progress of support plans per DDS regulations.
- Utilize the communication methods needed for assigned caseloads.
- Provide outreach and coordinate referrals to community services and other state agencies.
- Provide up-to-date record keeping – both electronic and hard copy; maintain documentation for waiver or non-waiver services.
- Familiarity with Microsoft programs, HCSIS and Meditech.
- Experience working with children and families with Intellectual Disabilities.
- Skills using communication styles and/or augmentative communication systems.

Schedule Information- Part time – 18.75-hour weekly position - 1st shift (Day Shift)
- Monday and Wednesday: 8:45am
-5:00pm and Friday 8:45 am to 12:30 pm - Days off – Sunday, Tuesday, Thursday and Saturday
Flexibility with work schedule will be required for this position.

Minimum Entrance RequirementsApplicants must have (A) at least three (3) years of full-time or equivalent part-time, professional experience in human services work or social work or (B) any equivalent combination of the required experience and substitutions below.
Substitutions- I. A Bachelor's degree or higher in a related field may substitute for two (2) years of the required (A) experience.
