Community Counseling of Bristol County (CCBC) in Brockton, MA, is seeking an entry-level Case Manager for the PACT‑Y program. This full-time role runs 11‑7, M-F with weekend rotation and some flexibility, serving youth and families in schools, homes, and the community.
As part of a multi-disciplinary team you will support independent living, employment, and community integration, coordinate care with DMH, and help youth practice skills and access local resources.
