Responsibilities
- Contribute to the holistic assessment, planning, delivery and evaluation of patient care in patients’ own homes.
- Implement, monitor and maintain high standards of care.
- Re‑assess and re‑evaluate programmes of care when needed.
- Act as an active member of the team, sharing best practice whenever possible.
- Support and assist with daily team coordination, including handover, allocation of work and managing referrals and telephone messages.
- Develop good working relationships with Allied Health Professionals and other clinical services within Bromley Healthcare.
- Contribute to the development of policies and procedures that cover related services.
- Safeguard children, young people and adults at risk.
Shift options: 08:15‑16:15 or 10:45‑18:45. Must work one weekend in four and may take additional evening and weekend shifts if desired.
Career Pathway and Band ProgressionAll Community Nurses start at Band
5. Upon successful completion of defined clinical and non‑clinical competencies, progression to Band 6 Senior Staff Nurse is automatic. From Band 6, the District Nursing course is available (subject to availability) and support is provided for those wishing to progress to a District Nursing qualification.
Our district nursing service offers specialist care in Tissue Viability, Bladder and Bowel, Rapid Response and Nursing Rehabilitation units.
- Current NMC Registration, or await confirmation of qualification and registration at time of application.
- Experience in holistic patient assessments and care planning.
- Experience (or willingness to attend study days) in administration of drugs.
- Knowledge of standard setting, clinical audit and community placement.
- Experience in teaching and assessing skills.
- Experience in caring for older people.
- Experience with PICC lines / IV access is desirable.
- Evidence of provision of high standard clinical care and accurate record keeping.
Valid driving licence and access to a vehicle for work purposes, with the ability to use a satnav.

Salary is dependent on NHS experience and current banding/pay point.
