As part of a dynamic team, you will work two days in the office per week, including a fixed team day on Thursday and a flexible second day of your choice.

What you will do:

  • Promote independence and wellbeing for adults and their carers
  • Safeguard individuals at risk and respond to safeguarding concerns
  • Assess needs in line with the Care Act 2014
  • Develop and deliver person-centred care and support plans
  • Work collaboratively with service users, carers, and partner agencies
  • Monitor and review care to ensure high-quality, effective support
  • Contribute to service development and continuous improvement
