Responsibilities include:

• Provide face-to-face and telephone consultations to patients
• Deliver structured medication reviews and medicines optimisation
• Carry out targeted reviews for high-risk and vulnerable patients
• Reconcile medicines following hospital discharge and clinic appointments
• Review medication for newly registered patients
• Implement and maintain robust repeat prescribing systems
• Lead on safe, effective, and cost-conscious prescribing
• Review and act upon medication safety alerts
• Support patients with self‑limiting and minor ailments
• Improve medicines compliance and patient understanding
• Provide expert medicines advice to clinical and non‑clinical staff
• Act as the practice lead for medicines management
• Support QOF, Enhanced Services, and PCN targets
• Contribute to audit, service evaluation, and quality improvement
• Review and implement NICE, CQC, and clinical guideline updates
• Provide education and training to the wider clinical team
• Work collaboratively within the multidisciplinary team
• Maintain high standards of confidentiality, professionalism, and clinical governance
• Promote equality, diversity, and inclusion across all aspects of care
• Adhere to health, safety, fire, and environmental regulations
• Participate in mandatory training, CPD, and professional development
• Support continuous service improvement and innovation
