The Home Health Manager provides mid-level operational and clinical oversight of the Home Health program. This role supports the Director of Nursing by supervising home health nursing and aide staff, ensuring regulatory compliance, reviewing OASIS documentation, coordinating interdisciplinary care, and maintaining efficient daily operations.

The Home Health Manager collaborates closely with the Therapy Clinical Manager, Intake Coordinator, QAPI, scheduling, and administrative leadership to ensure high-quality, timely, and compliant patient care
. The role serves as a liaison between the agency, physicians, hospitals, SNFs, and other community partners.

Essential Duties and Responsibilities
  • Provides day-to-day supervision of Home Health RNs, LPNs, and Home Health Aides.
  • Facilitates weekly Home Health Care Conferences and ensures care planning aligns with regulations and agency policy.
  • Reviews all OASIS assessments (SOC/ROC/Recert/DC) for accuracy, timeliness, and clinical completeness.
  • Conducts admission visits or other clinical visits as needed based on staffing and acuity.
  • Coordinates nursing care with the Therapy Clinical Manager to ensure interdisciplinary alignment.
  • Ensures timely documentation and addresses errors or coaching needs promptly.
  • Oversees weekly visit scheduling for nursing and HHA staff; approves PTO requests and manages staffing to ensure adequate coverage.
  • Oversees the on-call schedule for nursing (Home Health–specific), in collaboration with the DON.
  • Approves timecards and mileage for home health field staff.
  • Ensures timely ordering of clinical supplies and equipment required for Home Health patients; approves supply & Interpath lab ordering within agency guidelines.
  • Supports Intake in screening referrals and determining Home Health appropriateness.
  • Ensures continuous survey readiness and compliance with CMS Conditions of Participation and state regulations in conjunction with DON.
  • Participates in QAPI activities (Yes—continues involvement), including audits, documentation review, and performance improvement projects.
  • Monitors clinical productivity and caseload distribution to ensure efficiency and safety.
  • Participates in interviewing, hiring, onboarding, and competency validation of HH nursing staff (LVN/RN/HHA).
  • Conducts supervisory and ride‑along visits per agency policy.
  • Provides ongoing staff education, feedback, and performance evaluation.
  • Participates in progressive discipline with the DON when necessary.
  • Acts as liaison between Home Health and physicians, hospitals, SNFs, discharge planners, and other community providers.
  • Supports the DON and CEO with community outreach, presentations, and marketing as requested.
  • Participates as a member of the clinical management team in program planning, process improvement, budgeting input, and staffing decisions.
  • Provides limited on‑call backup support as needed (Yes—but reduced, primarily when DON is unavailable).
  • Performs other responsibilities as assigned by the DON or CEO.
  • Supervisory Responsibilities
    • Direct supervision of Home Health RNs, LPNs, and Home Health Aides.
    • Responsible for performance management, coaching, evaluation, and ensuring adherence to standards of care.
    • Works collaboratively with the DON on corrective action when required.
    Required Knowledge and Basic Skills Knowledge of
    • Nursing principles, practices, procedures, and nursing process.
    • Familiarity with and commitment to the agency mission.
    • Medical terminology, drug actions and interventions and medical record keeping practices.
    • Thorough understanding of family and team dynamics and the ability to relate supportively to both patient and family.
    • Medicare and licensure regulations, standards, policies and procedures as related to Home Health and Hospice services.
    • Principles of supervision and personnel practices, along with good verbal and written communication skills.
    Ability to
    • Accept responsibility, exercise leadership and provide care delivery management for the home health and telemonitoring programs.
