We are seeking a compassionate LPN to provide skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse (RN), following physician orders and individualized care plans.

• Provide nursing treatments and implement patient care plans.
• Monitor patients' conditions and report changes to the supervising RN and physician.
• Educate patients and families on care needs and treatment plans.
• Maintain accurate clinical notes and patient records.
• Assist RNs with specialized procedures and participate in patient care conferences and team meetings.
• Follow nursing standards, confidentiality policies, and applicable regulations.

• Graduate of an approved practical nursing program with an active Oregon LPN license.
• Current BLS certification.
• At least one year of nursing experience in home health, hospice, or a healthcare facility; three years preferred.
• Knowledge of nursing procedures, medications, pain management, and electronic medical records.
• Strong communication skills and ability to work independently.
• Valid Oregon driver's license, vehicle insurance, and a good driving record.

• Travel throughout the service area to visit patients in their homes.
• Ability to perform physical tasks, including walking, bending, climbing stairs, and lifting as needed.
• Flexibility to work occasional holidays and participate in on-call rotations.
