Monday – Friday with flexible schedules.

USD $35.00 – $40.00 per hour.

• Assess clinical nutrition needs of residents/patients and develop documented plans in the electronic medical record.
• Develop, implement, and evaluate individualized care plans based on clinical findings, lab results, weight trends, intake, and comorbidities.
• Provide nutrition education and counseling to residents/patients, families, and staff.
• Collaborate with interdisciplinary teams to determine a plan of care that improves residents' and patients’ overall health and well‑being.
• Ensure adherence to CMS, State, and Federal regulations and company standards.
• Provide coverage in multiple buildings based on company needs, and as desired.
• All other duties as assigned.

• Registered by the Commission on Dietetic Registration (RD).
• Hold a certification/license in good standing within the state of practice.
• Knowledge of the state survey process and compliance with nutrition care regulations (preferred).
• Strong organizational and communication skills.
• Positive attitude and effective communication with building occupants, administration, and HCSG leadership.
• Compliance with COVID‑19 vaccination policies as required.
• Multisite registered dietitian must have a valid driver’s license.
• RD‑eligible and provisional state licensure accepted in certain locations.

Salary determined by qualifications, experience, education, and facility needs.
