Position: Case Manager (Service Coordinator)

Position Summary

Provide Case Management services to individuals who have an intellectual disability and/or developmental disability. Primary goals: establish positive working relationships, explore options, design and monitor Person‑Centered Plans (PCPs), meet state‑mandated requirements, and secure support services.

Responsibilities
  • Advocate on behalf of individuals and provide resource information.
  • Educate individuals and families on self‑determination and service coordination.
  • Conduct Comprehensive Assessments (CA) and plan/update Person Centered Plans (PCPs).
  • Coordinate referrals, research and recommend resources to meet goals.
  • Travel to meet with support staff, conduct on‑site visits and comply with travel reimbursement policies.
  • Maintain HIPAA‑compliant records and use tracking & time‑management tools.
  • Submit required paperwork, such as Recertification of Need (RECON) and Agency Investigation Reports (AIR).
  • Collaborate with providers for transitions into community or state facilities.
  • Monitor performance utilization benchmarks and adhere to SCI policies and procedures.

Education Requirements
  • A bachelor’s degree from an accredited human services program; OR
  • A bachelor’s degree in a non‑human services field with two years of human services experience.
Experience Preferred

Experience working with people who have developmental disabilities.

Physical Demands

Standard office environment. Must remain stationary >95% of the time, operate computer and peripheral equipment, occasionally move office equipment weighing 10–25 lbs, drive a vehicle, and maintain close vision. Reasonable accommodations may be made.
