$70,500.00/yr - $96,900.00/yr

This range is provided by Center Well Home Health. Your actual pay will be based on your skills and experience — talk with your recruiter to learn more.

Pay range: $70,500 - $96,900 per year (base pay) or $45.00 - $63.00 per visit/unit.

• Provide admission, case management, and follow-up skilled nursing visits for home health patients.
• Administer ongoing care and case management for each patient, with follow-up as directed by the Clinical Manager.
• Confer with physician to develop the initial plan of treatment; provide hands-on care, management and evaluation of the care plan; revise plan in consultation with physician as needed.
• Coordinate care with various healthcare personnel (e.g., Physical Therapists, Occupational Therapists, Home Health Aides, and external providers).
• Report patient care/condition/progress to the patient’s physician and Clinical Manager on a continuous basis.
• Implement patient care plan with patient and family to address needs and goals.
• Coordinate/supervise Home Health Aides and Personal Care Workers; ensure current care plans and documentation are maintained.
• Discharge patients after consultation with the physician and Clinical Manager, and complete required clinical documentation.
• Prepare medical documentation for all patients, including case conferences, patient contacts, medication changes, re-certifications, progress updates, and care plan changes.

• Diploma, Associate or Bachelor Degree in Nursing
• Minimum of one year nursing experience preferred
• Strong med/surg, ICU, ER, and acute care experience
• Home Health experience a plus
• Current and unrestricted Registered Nurse licensure
• Current CPR certification
• Strong organizational and communication skills
• Valid driver’s license, auto insurance and reliable transportation
