  • Key responsibilities include comprehensive patient assessments in the home, development and revision of individualized plans of care, wound care and preventive care, medication management, IV therapy administration when permitted, infusion management, symptom monitoring, and clear patient and family education to support adherence and safety. Documenting care accurately, coordinating with physicians and therapists, and leveraging telehealth or digital health tools to monitor progress are integral parts of the role.
