Responsibilities

• Provide compassionate and skilled care to patients in their homes, including assistance with activities of daily living (ADLs), personal care, and homemaking tasks
• Develop and implement personalized care plans in collaboration with patients, families, and healthcare teams
• Monitor and report patient progress to the healthcare team, ensuring continuity of care and effective communication
• Build strong relationships with patients and families, promoting dignity, respect, and independence
• Stay up-to-date with the latest knowledge and skills in home health care, adhering to industry standards and regulations

Requirements

• High school diploma or equivalent required
• Completion of a recognized Home Health Aide training program
• Current certification as a Home Health Aide (HHA) or willingness to obtain within a specified timeframe
• Ability to work independently and as part of a team
• Strong communication, empathy, and problem-solving skills
• Reliable transportation and ability to travel to patient homes
