LPN​/RN Case Manager

LPN / RN Case Manager Brooklyn, NY

We are seeking a dedicated and detail-oriented Licensed Practical Nurse (LPN) or Registered Nurse (RN) – Case Manager to join our growing team. The Case Manager will coordinate care, ensure compliance, and support a caseload of patients receiving private duty nursing services. This role serves as a key point of contact for patients, families, and caregivers.

• Manage a caseload of patients receiving home care nursing services.
• Serve as a liaison between patients, patients’ families and insurance companies.
• Develop and update individualized care plans in collaboration with RNs and other healthcare professionals.
• Ensure documentation is complete, timely, and compliant with state and agency regulations.
• Monitor field nurses’ performance and provide clinical support or coaching as needed.
• Communicate regularly with patients and families to address concerns, care needs, and updates.
• Participate in interdisciplinary meetings and case conferences.
• Support emergency coverage planning or last-minute needs, including occasional home visits.
• Ensure all care aligns with agency standards, patient rights, and clinical guidelines.

• Current and valid LPN license in New York State.
• Minimum 2 years of experience in home care, private duty nursing, or related healthcare setting.
• Strong clinical knowledge and ability to manage medically complex cases.
• Excellent communication, organization, and time management skills.
• Comfortable with EMR systems and digital documentation.
• Ability to work independently and collaboratively in a fast-paced environment.
• Valid driver’s license and reliable transportation preferred for occasional field visits.
