POSITION OVERVIEW

A Licensed Practical Nurse (LPN) is responsible for LPN nursing care and medical procedures within the scope of the nursing practice. The LPN assists in the integration of physical and behavioral health in the program and reports to the RN and/or Nursing Leadership.

KEY ESSENTIAL FUNCTIONS
  • In line with doctor’s orders, administer prescribed and/or routine medical treatments while adhering to all internal policies on administration and documentation.
  • Assist with scheduling appointments and obtaining medical documentation prior to the client’s appointments.
  • Function as the liaison between external medical health care providers, schools, other community organizations, and the facility regarding the client’s medical needs.
  • Provide general nursing care within the scope of LPN practice, including:
    • Take and record client’s vital signs.
    • Administer prescribed/specified medication and note time and amount on the client’s chart.
    • Observe clients and report adverse reactions to medication or treatment to supervisor.
    • Perform phlebotomy and prepare specimens for laboratory submission.
    • Inventory and requisition supplies.
    • Respond to medical emergencies immediately, perform triage as necessary, stabilize the client, and provide necessary medical treatment within an LPN’s scope of practice. Follow up to ensure complete recovery.
    • Perform all required charting and patient documentation as mandated by internal practice and external regulatory bodies.
  • Carry out any additional tasks within the scope of an LPN NYS license requested by the RN and/or Nursing Leadership.

  • Communicate practice knowledge effectively, both orally and in writing.
  • Participate in agency quality improvement projects to ensure best client outcomes.
  • Create a climate of patient‑centered care, including confidentiality, privacy, comfort, emotional support, mutual trust, and respect.
EDUCATIONAL / TRAINING REQUIRED
  • Hold an active LPN nursing New York State license.
  • Maintain active CPR certification.
EXPERIENCE / LANGUAGE PREFERENCE
  • Excellent oral, written, communication, and organizational skills.
COMPUTER SKILLS REQUIRED
  • Excellent aural and written communication and collaboration skills.
  • Excellent organizational skills and data management capabilities.
VISUAL AND MANUAL DEXTERITY
  • Ability to read documents for analytical purposes such as computer information, software, and graphic design programs.
  • Limited applications of manual dexterity and hand‑eye coordination.
WORK ENVIRONMENT / PHYSICAL EFFORT
  • Ability to walk, sit, stand, and bend as required to assess and provide care to clients.
  • Capability to respond swiftly to emergencies, including moving quickly to the scene, performing CPR, and utilizing appropriate emergency protocols.
  • Proficiency in lifting, transferring, and repositioning clients safely when needed.
  • Maintain physical stamina to perform nursing duties throughout the shift, including prolonged periods of standing and walking.
  • Ability to navigate a variety of environments, including tight spaces, while maintaining a focus on patient care and safety.
