Director of Intake & Case Management

About the Role

A well-established Home Care agency is seeking a skilled and detail-oriented Director of Intake & Case Management to oversee the referral and onboarding process for new patients. This key leadership role involves managing the intake team, coordinating with nurses, insurance plans, and referral sources, and ensuring all cases move smoothly from referral to start of care.

• Supervise and lead the intake and case management team
• Accept and process incoming referrals from marketers, hospitals, and other sources
• Communicate with insurance plans to verify eligibility, secure authorizations, and follow up as needed
• Coordinate and schedule nurse evaluations and patient assessments
• Maintain close communication with nurses, patients, and referral partners to ensure timely and accurate service delivery
• Review and proof patient orders and documentation, including nightly checks when necessary
• Ensure compliance with all Home Care regulations and internal procedures
• Track and manage caseloads, ensuring all patients receive proper care coordination
• Support continuous process improvement within the intake and case management functions

• Proven experience in Home Care Intake and/or Case Management (required)
• Strong understanding of insurance processes, Medicaid/Medicare, and authorization procedures
• Excellent communication, leadership, and organizational skills
• Ability to multitask and work efficiently in a fast-paced environment
• Proficiency in EMR systems and Microsoft Office Suite
