RN Manager

Overview

Ultimate Care and Swift Home Care is seeking a dedicated RN Manager to oversee the clinical operations of the home care agency, ensuring compliance with New York State Department of Health and federal regulations. This role involves supervising nursing and paraprofessional staff, coordinating patient care, conducting quality assurance reviews, and supporting the Director of Patient Services in maintaining the highest standards of patient care and safety.

Schedule:

M-F, 9:00 AM to 5:30 PM, fully on-site.

Compensation: $110,000 - $120,000 annually

• Supervise and support Field RNs, including documentation review, training, orientation, and competency evaluations.
• Conduct clinical assessments for NHTD and TBI Waiver programs, participate in team meetings, and resolve incidents or complaints.
• Ensure compliance with NYSDOH, CMS, and agency policies through audits, surveys, and performance improvement initiatives.
• Promote quality assurance and risk management by identifying gaps in care, implementing corrective actions, and monitoring incident reports.
• Serve as a clinical resource and liaison for staff, patients, families, caregivers, and physicians.
• Assist the Director of Patient Services with additional projects as needed.

• Current New York State Registered Nurse (RN) license in good standing
• Minimum of 3 years of nursing experience, with at least 1–2 years in home care and/or NHTD Waiver preferred
• Excellent organizational, leadership, and communication skills
• Proficiency in EMR systems and Microsoft Office Suite
